Agenus, en colaboración con el Canadian Cancer Trials Group (CCTG), está llevando a cabo un ensayo mundial de fase 3 para evaluar la combinación de inmunoterapia de botensilimab (BOT) y balstilimab (BAL) frente a la mejor atención de apoyo (BSC) en pacientes con cáncer colorrectal refractario, irresecable y estable en microsatélites (MSS) o con capacidad de reparación de desajustes (pMMR). Este estudio de registro se llevará a cabo como un ensayo grupal cooperativo internacional, dirigido por CCTG y respaldado por redes académicas como AGITG (Australasia) y PRODIGE (Francia), que comprende Unicancer, GERCOR y FFCD. ROBBIN is a planned global, randomized, open-label Phase 3 study evaluating botensilimab plus balstilimab as neoadjuvant treatment for patients with previously untreated, resectable, high-risk Stage II or Stage III microsatellite-stable/mismatch repair-proficient colon cancer.
Approximately 850 patients are expected to be randomized 1:1 to receive either:
Botensilimab plus balstilimab before surgery, followed by standard-of-care adjuvant chemotherapy or observation based on pathologic stage and clinical guidelines; or surgery followed by standard-of-care adjuvant chemotherapy or observation.
The primary endpoint is event-free survival. Key secondary endpoints include overall survival, preservation of circulating tumor DNA clearance and quality of life. The study will also evaluate pathologic response, the relationship between pathologic response and longer-term outcomes, use of adjuvant chemotherapy and potential biomarkers.
The ROBBIN study is planned and has not yet begun enrollment. Study details remain subject to final protocol completion and regulatory and ethics approvals.
This study is an open-label, Phase 1, multicenter study to evaluate the safety, tolerability, pharmacokinetics (PK), and pharmacodynamic (PD) profiles of a novel fragment crystallizable (Fc)-engineered immunoglobulin G1 anti-cytotoxic T-lymphocyte antigen 4 (anti-CTLA-4) human monoclonal antibody (botensilimab) monotherapy and in combination with an anti-programmed cell death protein-1 (PD-1) antibody (balstilimab), and to assess the maximum tolerated dose (MTD) in participants with advanced solid tumors. This study will also determine the recommended phase 2 dose (RP2D) of botensilimab monotherapy and in combination with balstilimab.
Se trata de un estudio abierto, de fase 2 y multicéntrico para evaluar la eficacia, la seguridad, la tolerabilidad y los perfiles farmacocinéticos del botensilimab en monoterapia y en combinación con balstilimab o los tratamientos estándar en participantes con cáncer colorrectal metastásico resistente al tratamiento.
